Description
Gemtuzumab ozogamicin, also known as Mylotarg, is a fusion molecule that serves as an antibody-targeted antineoplastic agent. It is a humanized mouse monoclonal antibody (IgG4) derived from the CD33 antigen, which is a sialic acid-dependent adhesion protein. This protein is expressed on the surface of leukemia blasts and immature normal cells of myelomonocytic origin but not on normal hematopoietic stem cells or nonhematopoietic cells. Gemtuzumab ozogamicin is linked to the cytotoxic antibiotic calicheamicin via a bifunctional linker. When the anti-CD33 antibody binds to its antigen, the complex is internalized, and the calicheamicin derivative is released inside the cell, causing double-strand breaks and cell death.

Gemtuzumab is used as a treatment for patients with CD33-positive acute myeloid leukemia (AML) in first relapse. It is particularly indicated for adults aged 60 years or older who are not considered candidates for cytotoxic chemotherapy. The drug has demonstrated an acceptable safety profile and has achieved meaningful clinical remission in 32 to 43% of individuals, making it at least as effective as conventional therapy with a more favorable tolerability profile compared to standard chemotherapeutic drugs used to treat AML.
