Ezetimibe is a new class of selective cholesterol absorption inhibitors, which can inhibit the absorption of cholesterol from the small intestine by binding to the intestinal brush-like marginal vesicle superior membrane protein (relative molecular weight 145X103), and reduce the cholesterol content in serum and liver. Unlike cholic acid mixture, ezetimibe did not affect the absorption of cholesterol esters, other steroids (such as bovine cholic acid), triacylglycerol and fat-soluble vitamins. Its pharmacological effect has nothing to do with the inhibition of acetyl-coenzyme A-cholesterol acetyltransferase (ACAT) and the expression of LDL receptor (scavenger receptor). After absorption, ezetimibe was bound to glucuronic acid in the liver and circulated through the liver and intestine, and was almost specifically localized in small intestinal mucosal cells.
