Methazolamide is also a carbonic anhydrase inhibitor and is mainly used in the treatment of glaucoma. It is a homologue of acetazolamide, and its chemical structure is similar to acetazolamide (one more methyl group on the nitrogen atom). Therefore, its pharmacological action and mechanism of action are the same as those of acetazolamide. The difference in structure reduces ionization, so the intraocular permeability is enhanced compared with acetazolamide. The effect of this product to penetrate the blood-aqueous humor and blood-brain barrier is also stronger than that of acetazolamide (the concentration of human cerebrospinal fluid is 50 times higher than that of acetazolamide). The inhibitory effect of carbonic anhydrase is 60% stronger than that of acetazolamide, and only 55% of methazolamide is bound to plasma proteins in the body, so lower doses have obvious ocular hypotensive reactions.

For primary open-angle glaucoma, angle-closure glaucoma and some secondary glaucoma, and the adjuvant treatment of patients with unsatisfactory intraocular pressure control with topical anti-glaucoma drugs. Because this product lowers intraocular pressure and has less effect on acid-base balance, it is better than acetazolamide for patients with severe obstructive pulmonary disease. The effect on urinary citrate secretion is smaller than that of acetazolamide, so methazolamide is recommended for patients who need oral carbonic anhydrase inhibitor treatment but are prone to kidney stone formation.
