Diethylene glycol is a chemical that is readily available and has been implicated in multiple mass poisonings when used as a diluent in pharmaceutical preparations. Diethylene glycol is found commonly in various chemicals including automotive products and wallpaper stripper. It has two ethylene glycol molecules with a stable ether bond and is metabolized by alcohol dehydrogenase to two known toxic metabolites, 2-hydroxyethoxyacetic acid and diglycolic acid. It causes an anion gap acidosis, cortical necrosis, and neurotoxicity. Despite it being very commonly available, many questions remain about its toxicologic fate and mechanism of toxicity.
