Pimecrolimus is a semi-synthetic, macrocyclic lactone derived from ascomycin by activation of the 32-hydroxy group with a triflate ester, and nucleophilic substitution with chloride under phase transfer conditions to provide the chloro analogue. Pimecrolimus has been targeted for treatment of inflammatory skin disorders. Like all tacrolimus analogues, pimecrolimus binds to receptor protein, FKBP12. The complex then binds to mTOR preventing it from interacting with target proteins. Pimecrolimus is extensively cited in the literature with over 2,000 citations.
 

In vitro studies

Pimecrolimus blocks the activation pathway of T lymphocytes by inhibiting the function of phosphatase. 
Pimecrolimus prevents the release of cytokines and pro-inflammatory mediators from mast cells. 
Pimecrolimus binds to macrophilin-12 to form a pimecrolimus-macrophilin complex, which then binds to the cytoplasmic enzyme calcineurin. 
Pimecrolimus-macrophilin complex Chemicalbook compound inhibits the action of calcineurin to prevent the dephosphorylation of the cytoplasmic components of nuclear factors in activated T cells. 
Pimecrolimus not only inhibits the transcription and synthesis of cytokines in mast cells, but also inhibits the release of pre-formed mediators serotonin and β-hexosaminidase by inhibiting Fcε-RI-mediated degranulation and secretion. 
Pimecrolimus treatment caused a strong down-regulation of the mRNA expression of genes related to the macrolactam target pathway and inflammation.
