In biochemical tests, it is often seen that after centrifugation of the blood collecting vessel equipped with serum separation gel , the serum in the upper layer is red, which is the rupture of blood cells in the test tube and the escape of hemoglobin. Outside the human body, if the red blood cells are in a solution with low osmotic pressure, water enters the red blood cells, causing expansion and rupture, resulting in hemolysis. Hemolysis in human body is mainly due to the internal defects of red blood cells, or the excessive destruction of red blood cells due to the presence of autoantibodies, chemicals, snake venom and other factors in plasma. So why does hemolysis occur in the test tube containing serum separation gel
First, let's understand the principle of separating serum and blood clot with lower separating gel. After the blood sample is put into the test tube, under the action of thrombin factor, fibrinogen is transformed into insoluble fibrin filaments, which surround the blood cells to form blood clots, which naturally coagulate and precipitate part of the serum, but it is difficult to fully coagulate. After the separation gel is added, because the separation gel has thixotropy, it becomes liquid under the action of centrifugal force and flows in the test tube. Due to the specific gravity, the blood clot with high specific gravity precipitates at the bottom, the small serum is in the upper layer, and the separation glue is in the middle. After the centrifugal force disappears, the separation gel becomes colloidal and completely separates the blood clot from the serum.
The separating gel is inert and does not react with any substance in the chemical reaction. Therefore, it will not change the product of the reaction or destroy the hemoglobin in the cell. From this point of view, serum separation gel is not the reason for destroying blood cells. So what are the other factors causing blood cell damage
1. Disinfect with Iodophor. The alcohol in iodophor is not dry and enters the blood collection vessel, causing pollution and damaging red blood cells;
2. Blood collection: if the blood collection volume is insufficient and the negative pressure is too large, the red blood cells in the tube will be broken, which will cause hemolysis;
3. Transfer the blood sample to the blood collection vessel with a blood collection needle, and destroy the blood cells due to the effect of pressure;
4. Turn the blood collection vessel upside down and mix it for about 5 times. If the force is too strong, the blood cells will rupture;
5. Allow the blood to coagulate naturally after standing for 30 minutes. If the time is not fully reached, the blood will be centrifuged before coagulation;
