1. The overview of caspofungin
Caspofungin (CAS NO.179463-17-3) is a lipopeptide antifungal drug. It for injection was originally approved by both the Food and Drug Administration (FDA), in the US, and the EMEA, in Europe, in 2001. It is a member of a new class of antifungals termed the echinocandins. Caspofungin is usally administered intravenously.
2. Pharmacologic action of Caspofungin
Caspofungin is half synthetic lipopeptide (echinocandin) compound. The echinocandin lipopeptide caspofungin is the first of a new class of antifungal compounds that inhibit the synthesis of 1,3-β-D-glucan. This homopolysaccharide is a major component of the cell wall of many pathogenic fungi and yet is absent in mammalian cells. It provides osmotic stability and is important for cell growth and cell division.
In vitro, caspofungin has broad-spectrum antifungal activity against Candida and Aspergillus spp without cross-resistance to existing agents. The compound exerts prolonged post-antifungal effects and fungicidal activity against Candida spp and causes severe damage of Aspergillus fumigatus at the sites of hyphal growth.
Animal models have demonstrated efficacy against disseminated candidiasis and disseminated and pulmonary aspergillosis, both in normal and in immunocompromised animals. Caspofungin possesses favourable pharmacokinetic properties and is not metabolised through the cytochrome P450 (CYP) enzyme system.
Tips: Caspofungin has been shown in animal studies to have embroyotoxic properties, and therefore has been assigned to class C. It should only be given to pregnant women if the benefit to the mother clearly outweighs the potential risk to her fetus. The drug is found in the milk of lactating rats, but it is not known whether this is seen in humans. Thus, lactating women should be treated cautiously.
